Education & Training
Dr. Jacqueline To is a Stanford- and UCSF-trained primary care physician dedicated to relationship-centered, compassionate medicine. Board-certified in internal medicine, she brings a strong background in primary care, interprofessional team-based care, and health systems leadership. Her north star is to provide compassionate medicine and partner with her patients to navigate complex medical issues that may arise at varying stages of life. She has received numerous awards for excellence in primary care, leadership, and research, including the Stanford Firestone Medal, the Gregg Stracks Award for Leadership in Primary Care, the UCSF Alanna McKelvey Stone Compassion Award, and induction into the Gold Humanism Honor Society.
Dr. To discovered her passion for primary care and medicine as an undergraduate at Stanford University, where she served as a Clinic Manager at Pacific Free Clinic, caring for uninsured patients. A study-abroad term at the University of Oxford ignited an interest in innovative care delivery and health policy, through comparative analysis of U.S. and U.K. health systems.
After graduating with Honors from Stanford, she attended the University of Colorado School of Medicine, where she co-founded the D.A.W.N. Free Clinic – an interprofessional, student-run clinic providing essential primary care services to uninsured residents of Aurora, Colorado.
She returned to California for internal medicine residency at UCSF in the UCPC Primary Care track and Health Systems Leadership pathway, receiving specialized training in primary care, healthcare systems improvement, health policy, geriatrics, palliative care, and housecalls/home-based medicine. In 2021, she joined the Palo Alto Medical Foundation, where she cared for thousands of patients across the full spectrum of life, with patients ranging in age from 18-109.
Dr. To’s Approach
Dr. To’s goal is to ensure every patient feels respected, heard, and empowered in their own health. Because each individual is unique, she values the collaborative process of creating shared care plans that are medically sound, enhance quality of life, and honor personal values and preferences. She is committed to delivering evidence-based medicine and thoughtfully leveraging innovations in medical technology and research to provide comprehensive, team-based care. Above all, she strives to build long-lasting, meaningful relationships with every patient she serves.

Outside of medicine, Dr. To enjoys creative writing, podcasts, hiking, traveling, and exploring kid-friendly Bay Area haunts with her husband and toddler.
